Community First Services & Supports (CFSS) is a flexible, person-directed service designed to help individuals with disabilities or chronic health conditions live with greater comfort, stability, and independence in their homes and communities. At US Home Care LLC, we take a dedicated, thoughtful approach to CFSS by creating individualized support strategies that reflect each person’s goals, routines, and preferred lifestyle. The purpose of CFSS is not simply to provide assistance but to help individuals maintain meaningful daily experiences while receiving the practical help they need.
CFSS begins with an individualized assessment that identifies the daily tasks, personal needs, and environmental factors affecting the client. From there, a customized service plan is built to outline the type and amount of support needed. This may include hands-on help with bathing, grooming, dressing, toileting, meal preparation, and personal hygiene—tasks that support physical well-being and daily comfort. Each caregiver is trained to provide this assistance with sensitivity, patience, and respect for personal boundaries.
One major advantage of CFSS is its flexibility. Clients may choose a traditional provider-managed model or a budget model that allows them to select and manage their own workers. US Home Care LLC supports both approaches, ensuring clients receive guidance, training, and ongoing assistance to make the model successful for them.
