These Terms and Conditions (“Terms”) govern the use of services and website of US Home Care LLC (“we,” “our,” or “us”). By accessing our website or receiving services, you agree to these Terms.
US Home Care LLC provides home care and community-based services in accordance with Minnesota Department of Human Services (DHS) licensing requirements, including applicable 245D standards.
Services may include, but are not limited to:
Personal Care Assistance (PCA)
Individualized Home Support
Homemaker and Companion Services
Respite Care
Assisted Living Support Services
Services are provided based on assessed needs, eligibility, authorization, and available staffing.
Services are provided only to individuals who:
Meet DHS eligibility requirements
Have a valid service authorization or private-pay agreement
Provide accurate and complete information
US Home Care LLC reserves the right to decline or discontinue services if eligibility or authorization requirements are not met.
Individuals receiving services have the right to:
Be treated with dignity and respect
Receive services in a safe and person-centered manner
Participate in service planning
Privacy and confidentiality of personal information
File complaints without retaliation
Individuals and/or their representatives agree to:
Provide accurate and timely information
Treat staff respectfully
Maintain a safe service environment
Notify us of changes in health, address, or service needs
Service schedules are arranged in advance
Changes or cancellations should be reported as soon as possible
Repeated cancellations, no-shows, or unsafe conditions may result in service modification or termination
Services may be billed through DHS programs, insurance, waivers, or private pay
Private-pay clients are responsible for agreed-upon fees
Late or unpaid balances may result in suspension of services
US Home Care LLC:
Does not provide medical diagnosis or emergency medical treatment
Does not replace licensed medical or nursing care unless authorized
Is not responsible for outcomes beyond the scope of authorized services
In emergencies, individuals should call 911 or contact their healthcare provider.
We comply with HIPAA, Minnesota data privacy laws, and 245D confidentiality requirements. Use of personal and health information is governed by our Privacy Policy.
US Home Care LLC follows the Minnesota Maltreatment of Vulnerable Adults Act and is required to report suspected abuse, neglect, or exploitation.
Individuals agree to maintain a safe environment for staff. Services may be paused if conditions are unsafe.
Individuals have the right to file complaints or grievances without retaliation.

County or lead agency
Minnesota Department of Human Services

We do not guarantee uninterrupted or error-free website access
To the extent permitted by law, US Home Care LLC is not liable for indirect, incidental, or consequential damages related to service use or website access.
Services may be terminated:
At the individual’s request
If DHS authorization ends
For non-payment (private pay)
For unsafe conditions or violation of these Terms
Reasonable notice will be provided when possible.
US Home Care LLC may update these Terms to reflect DHS regulations or operational changes. Updated Terms will be posted with a revised effective date.
These Terms are governed by the laws of the State of Minnesota.
